TELK conducts medical evaluations for individuals on the following grounds:

  • Temporary incapacity for work

  • Permanent reduced work capacity or degree of disability

  • Other grounds provided for in regulatory acts

TELK also evaluates individuals regarding appealed sick leave certificates, which is handled based on the region of the medical facility that issued the sick leave.

Documents for TELK

The evaluation of permanent reduced work capacity is carried out by TELK (Territorial Expert Medical Board) and NELK (National Expert Medical Board). Preparing individuals with disabilities for appearance before the TELK commissions is carried out by their general practitioner (GP).

First, the patient must consult their GP, who will explain whether a TELK evaluation is possible based on their medical conditions and their severity. The GP determines the required medical documents, examinations, consultations, and lab tests needed for the individual, and provides them with a medical referral for TELK. With this referral, the patient submits their documents to the Regional Health Inspectorate (RHI) corresponding to their permanent address. If the individual is currently employed, an occupational profile report (proizvodstvena harakteristika) issued by the employer must also be attached.

Are there fees for preparing documents for TELK/NELK evaluation?

Health-insured individuals do not pay for the medical documents issued to them. They only pay the doctor the standard statutory co-payment (consumer fee) for the examination, unless they are exempt from it.

Patients do not pay any fee for the issuance of the referral by which the GP refers a health-insured individual for evaluation by TELK.

Information regarding collection of completed protocols – Monday 11:00 AM - 1:00 PM.

  1. Upon initial appearance before TOLEK – medical certificate for a motor vehicle driver, issued by a personal physician – containing only the individual's details and the personal physician's details with their stamp and signature.

  2. Upon each subsequent appearance before TOLEK – protocol from a previous decision of the commission.

  3. Green form (referral/certificate) from the personal physician describing the patient's health status.

  4. Green form from the general practitioner stating that the patient does not take intoxicants and is mentally healthy.

  5. Epicrises /from hospital facilities/, medical documentation /consultations, examinations/ TELK – copy.

  6. In case of diabetes mellitus – blood sugar profile.

  7. In case of diabetes mellitus – outpatient record from an eye examination dated no more than 6 months ago.

  8. In case of cardiovascular disease – ECG taken within a week of the commission date – outpatient record from a cardiologist.

  9. In case of epilepsy – stage epicrisis from the treating neurologist and a current EEG.

  10. In case of mental illness – stage epicrisis from the treating psychiatrist.

  11. Certificate of mental health /for drivers possessing professional categories/ and a job description from the employer.

  12. In case of glaucoma – outpatient record, computer perimetry, /KUGEL/ glaucoma program dated no more than 6 months ago.

  13. ID card and driver's license – copy.

Assoc. Prof. Georgi Iliev, MD

Assoc. Prof. Georgi Iliev is an established specialist in Otorhinolaryngology (ENT) with extensive clinical, academic, and surgical experience. His practice covers the full spectrum of diagnostics, state-of-the-art conservative treatments, and surgical procedures for disorders of the upper respiratory tract, auditory system, and neck in both adult and pediatric patients.

Dr. Nikolay Georgiev is a specialist gastroenterologist in Varna with over 10 years of medical experience. Biography

Dr. Georgiev is a frequent guest lecturer, delivering training courses for physicians, pharmacists, and dentists. Since 2019, he has maintained strong scientific interests, research projects, and publications in the fields of food allergies, food intolerances, and the gut microbiome.

He served as a faculty lecturer teaching both Bulgarian and international medical students at the Medical University of Varna until January 2022. Over the course of his career, he has been part of the medical teams at Medical Center "Klinika Nova Varna", UMBAL "St. Marina" Varna, Medical University of Varna, Medical Center "Hygia Varna", and MBAL Varna.

Since 2023, Dr. Georgiev has been serving as the Head of the Department of Gastroenterology and Hepatology at MBAL Varna - Transport Hospital.

Dr. Georgi Kesov

Dr. Georgi Kesov, MD, PhD is a specialist in general and thoracic surgery based in Sofia. His professional interests focus on providing comprehensive solutions for oncology patients, expeditionary medicine, and medical management in resource-limited environments. He performs Doppler sonography of peripheral vessels, abdominal organs, superficial structures, breasts, muscles and joints, the urogenital system, as well as thoracic/pleural and thyroid imaging. He is the recipient of several international NATO and EU awards for his innovative work. His clinical focus includes breast surgery, thyroid surgery, and laser aesthetic surgery.

  • 1998: Graduated in Medicine from the Medical University of Sofia.

  • 2007: Acquired his specialty degree in General Surgery from the Medical University of Plovdiv.

  • 2014: Acquired his second specialty degree in Thoracic Surgery from the Medical University of Sofia.

From 2008 to 2014, Dr. Kesov served as an Assistant Professor in the Department of Thoracic Surgery at the Military Medical Academy (MMA), and for the following 4 years, he headed the Breast Surgery Unit within the same department. In 2017, he served as Chief Surgeon at the DSC EUTM Surgical Hospital in Mali, and until 2021, as Chief Surgeon at the KFOR Surgical Hospital in Pristina, Kosovo. Until 2025, he served as a trusted physician for EU diplomatic missions, embassies, and consular services of EU member states, the UK, Japan, and others in the Republic of Kosovo. He has also participated in numerous overseas missions with the Bulgarian Armed Forces and the EU. He is fluent in English, Serbian, and Russian.

Dr. Nikolay Mladenov

Dr. Nikolay Mladenov, MD, PhD is a specialist in anesthesiology and intensive care. He graduated in Medicine from the Medical University of Varna "Prof. Dr. Paraskev Stoyanov" in 2013. In 2018, he acquired his specialty degree in Anesthesiology and Intensive Care.

In 2021, he was awarded the educational and scientific degree of Doctor of Philosophy (PhD) in Medicine after successfully defending his doctoral thesis titled "Ultrasonic Evaluation of the Optic Nerve in Patients with Clinical and Imaging Evidence of Increased Intracranial Pressure".

He has served as a Chief Assistant Professor at the Medical University of Varna "Prof. Dr. Paraskev Stoyanov", leading academic courses in Anesthesiology and Intensive Care and Emergency Medicine for both Bulgarian- and English-language medical programs.
